- Consultation QA Pipeline. A Zoom webhook starts a Gemini grader that scores every consult against the CEO's seven criterion rubric, with a transcript quote and timestamp behind each score. 471 consults scored live from May 19 to Oct 28 across three offices; median 2.7 minutes from transcript to sent report in October. It also drafts the doctor's chart note, guarded so a truncated note never ships.
- Pipeline observability hardening. Runs went green while emails never sent. I moved the run row to the top, made every failure retry or turn amber, resent 13 runs that had scored but never emailed, and added a Logs tab anyone can read.
- NPC Coach. Rebuilt an n8n call grader into a coaching platform for the front desk: it finds new patient calls among thousands of recordings, grades each on six criteria with a transcript quote behind every score, and tracks each coordinator against their own 30 day average. 690 real calls graded by October 29; in October the median grade came 14 minutes after the call ended. Live calls 16 days after the first commit.
- CBCT Scan Validator. In house dental CT quality gate, built in 37 days against a six figure vendor quote. Frozen DentalSegmentator encoder on OpenVINO CPU, a cost tuned threshold, SFTP both ways with the lab system. Honest 0.63 AUROC on a strict holdout; our testing team has uploaded 123 studies to its review app.
- Treatment Estimator. Next.js and Postgres quoting tool for three offices. Every saved line keeps its price, tier, label and price book version, so a later price change never reaches a saved quote. v1.0.0 shipped 14.5 hours after the first commit.
- Cowork Dashboard. Monday.com boards turned into the leadership dashboard, moved from Apps Script to Cloud Run in September with one definition per metric. Fixing the patient to lead join linked almost every patient to a lead, and cash flow now reads Monday every morning.
- Accounting Automation Suite. Our controller's nine Python scripts moved from a laptop terminal to one page behind IAP in one evening. Every question is asked before the run, and every script edit is tested before it is committed.
- Reconciliation. A deterministic engine that cross checks the schedule, production ledger, and clinical notes across Eaglesoft and Denticon to confirm every patient seen got documented and billed correctly, surfacing only the exceptions. About 70 hours a week of manual cross checking by the insurance team became an exception review; live on Cloud Run.
- Centralized Diagnostic Filter. Intake and experience surveys with Gemini writing the doctor's page and a panorex read under the founder's own ruleset. Every Gemini call falls back through newer models, and clinic emails wait for both reports.
- Platform map. 45 live services in 14 Google Cloud projects for three offices and a lab. One service holds the Denticon credentials, the apps that need it read the mirror through a registry, and six apps file feature requests through one relay.
